You can make an appointment by calling our office or using our secure Patient Portal. Patients wishing to use the Prime Care Patient Portal for the first time should contact our office for log in information and instructions.
We ask that all new patients arrive 30 minutes prior to their scheduled appointment time. Please bring your insurance cards and all prescription medication bottles that you are currently taking with you. For your convenience, you can download our Patient Registration forms in advance. If you are unable to keep your appointment, kindly give 24 hours notice. Failure to show for your appointment or give adequate notice may result in a $30.00 charge.

If you have an emergency matter after hours and need to speak with the provider on call, please call 804-965-7965 and the operator will put you in contact with our on-call provider. We contract with MediCopy for the copying of medical records, FMLA and Disability forms. Due to the time involved there is a fee for this service. Please read the release carefully when you sign it.
