Appointments
Our office hours are Monday through Friday from 7:00 am to 6:00 pm. Patients may use our secure Patient Portal to request appointments online or call our office. Current patients wishing to use the Patient Portal for the first time should contact our office for log in information and instructions.
New Patients
We ask that all new patients arrive 30 minutes prior to their scheduled appointment time. Please bring your insurance cards and all prescription medication bottles that you are currently taking with you. For your convenience, you can download our Patient Registration forms in advance. Please fill them out and bring them with you to your appointment. Download forms here.
Sick or Urgent Visits
To schedule an appointment, please call 804-526-1111, option 1. We make every effort to accommodate requests for same-day and next-day sick visit appointments.
Please note that complex portal messages that involve medical decision-making may be billed to your insurance as a “virtual e-visit”. We will alert you in advance if this will be required. A small copay may apply for the the “virtual e-visit” depending on your plan. Please proceed to ER if you have a life threatening emergency.
Routine Visits
These are for non-urgent, chronic medical conditions and medication refills. Please call well in advance to get the appointment time you prefer with the Prime Care healthcare provider you usually see. We recommend that you make your next follow-up appointment before leaving the office.
Physicals/Annual Gynecological Exams/Well Child Exams
Please call in advance to schedule any of these exams to ensure you get the appointment time you prefer.
Cancellations
If you are unable to keep your appointment, kindly give 24 hours notice. Failure to show for your appointment or give adequate notice may result in a $30.00 charge.
After Hours Emergencies
If you have a life-threatening emergency, please call 911. If you have an emergent matter after hours and need to speak with the provider on call, please call 804-965-7965 the operator will put you in contact with our on-call provider. Please note that our on-call provider does not dispense medication and is not to be used for medication refills.
Letters, Forms and Medical Records
Your health care provider may fill out necessary form(s) that you need.
We contract with MediCopy for the copying of medical records, FMLA and Disability forms. Due to the time involved there is a fee for this service. Please read the release carefully when you sign it.
You may request records through our office or online at medicopy.net/patients where you can also learn about the release of information form completion processes or company contact information.

Pediatric Immunization Policy Statement
Our practice is committed to providing safe, high‑quality medical care and protecting the health of all patients and families we serve. Vaccination is a critical component of preventive healthcare and a shared responsibility between healthcare providers and families.

Vaccines are widely recognized as safe and effective in preventing serious, potentially life‑threatening diseases. We understand that parents want what is best for their children, and we share that goal. Our practice serves vulnerable individuals, including infants too young to be fully vaccinated, elderly patients, and those with weakened immune systems. Maintaining high immunization rates helps protect all patients and reduces the risk of disease transmission in our office.
Immunization Requirements
Our practice follows immunization schedules and guidelines established by the American Academy of Pediatrics. All patients are required to receive recommended immunizations unless a documented medical contraindication exists.
Parental Choice and Practice Policy
While we respect parental involvement in healthcare decisions, adherence to this immunization policy is a condition of receiving care in our practice. Families who choose not to follow recommended immunization schedules should seek care elsewhere.
